Authorities in Kosciusko County are investigating a fatal two-vehicle crash that occurred late Thursday morning near Milford, resulting in the deaths of two individuals. The collision, which took place on State Road 15, approximately a quarter mile north of Waubee Road in Van Buren Township, involved a head-on crash between a Chevy Silverado and a Hyundai Accent.
The driver of the Silverado, 68-year-old Dale E. Wert from Goshen, was critically injured and transported to Lutheran Hospital in Fort Wayne. Despite efforts to save him, Wert succumbed to his injuries. The driver of the Hyundai Accent, 19-year-old Sarah Mischke of Milford, was pronounced dead at the scene by the Kosciusko County Coroner’s Office due to the severity of her injuries.

Emergency responders from multiple agencies, including the Milford Police Department, Milford Fire Department, and local EMS, quickly arrived at the scene. Investigators from the Kosciusko County FACT Team are working to determine the cause of the collision.
